THE DECISION IN BRIEF
The stem stays in the bottle but the decorative top comes off. Is the bore too tight?
Preserve the separated parts and review both the top–stem connection and the opening load. A difficult extraction can expose a weak connection, but the failure alone does not identify which specification is wrong.
Keep the fracture evidence
Photograph the top, stem and bottle before further handling. Record whether the event occurred during first opening, after storage or following repeated use. Avoid cleaning or scraping the joint before the supplier has agreed what evidence is needed.
Retain the bottle with the failed assembly. A replacement bottle with a different bore cannot reproduce the same opening condition. If several decorative-top variants share a stem, keep their identities separate during the investigation.
Review two interfaces
Ask for the proposed top–stem construction and the bottle-bore assessment. Do not assume a visible metal or wood top tells you how the connection is made. Any change in adhesive, insert or assembly route needs its own supplier review.
Compare insertion, opening and the integrity of the top as separate observations. A stem that extracts comfortably still needs a secure top, while a strong joint does not make an excessively difficult opening acceptable. Do not choose a smaller stem without re-evaluating sealing.
Release the revised combination
Agree the evaluation sequence with the closure supplier, using the intended liquid and bottle. Include relevant storage and repeated opening where they belong to the use case. Record the accepted construction revision and any remaining restrictions.
Identify the revised top–stem construction in the sample and quotation records. If only the bottle or stem was changed, say so. The next delivery must be inspectable against the correction that was actually evaluated, rather than a broad claim of improved strength.
